Summary

The facility's Quality Assurance Committee failed to ensure the effectiveness of the plan of correction for previously identified deficiencies, as evidenced by repeat citations during the most recent annual recertification survey. Specifically, deficiencies F812, F880, and F883 were cited again for the same issues as the prior survey. F812 involved failure to maintain the kitchen in a clean and sanitary manner and to discard expired foods. F880 was cited for failure to implement a water management program to monitor for and prevent the growth and spread of Legionella and other water-borne pathogens.
