Summary

Staff failed to provide wheelchair foot pedals for a resident with multiple medical conditions, including muscle weakness, unsteadiness on feet, and impaired upper extremities, while she was being pushed in the hallway. Observations showed that the resident was transported by both a licensed nurse and a certified nurse's aide without foot pedals attached to her wheelchair, and staff asked her to hold her feet up during transport.
