Summary

A deficiency occurred when a resident was not free from significant medication errors due to the facility's failure to ensure that admission medications were properly transcribed and administered. The resident, a female with multiple diagnoses including stage 2 pressure ulcer, chronic kidney disease, hypertension, type 2 diabetes, lymphedema, COPD, cellulitis, and morbid obesity, was admitted to the facility. On the day following her admission, it was discovered that the admission process had not been completed by the nursing staff. Review of the Medication Administration Record confirmed that none of the resident's daily medications, including amlodipine, furosemide, cetirizine, duloxetine, carvedilol, famotidine, fluticasone inhaler, heparin, hydralazine, and insulin lispro, were administered on the day in question. The facility's policy required careful review and transcription of physician orders upon admission, but this process was not followed, resulting in the resident missing essential medications.
