Summary

The facility failed to ensure that a Registered Nurse (RN) was on duty for eight consecutive hours each day, seven days a week, during the period from 4/6/25 to 4/12/25. Review of the staff schedule for 4/5/25 - 4/6/25 showed that all nursing shifts were staffed by Licensed Practical Nurses (LPNs), and the only RN listed had their name crossed out.
