Summary

A deficiency occurred when a resident with Alzheimer's disease, generalized muscle weakness, and difficulty walking was not provided adequate assistance during a bed linen change, resulting in the resident rolling out of bed and sustaining a left hip fracture. The resident was assessed as having severely impaired cognition, was non-ambulatory, always incontinent, and required maximal assistance with bed mobility and total care for activities of daily living. An x-ray revealed an acute fracture of the left femur with mild displacement, and the resident was subsequently transferred to the hospital for evaluation. The incident was documented as unwitnessed by some staff, but the nurse aide involved reported witnessing the fall during care.
