Summary

The facility failed to maintain complete and accurate medical records for a resident who was sent to the hospital for a CT scan. Specifically, there was no documentation in the resident's medical record regarding the transfer to the hospital for the CT scan, nor was there a physician's order for the procedure recorded. Facility policies reviewed indicated that verbal orders must be recorded immediately in the resident's chart and that all services provided, as well as changes in the resident's condition, should be documented to facilitate communication among the care team. The administrator acknowledged that documentation of the order and the resident's transfer was necessary for continuity of care and to ensure follow-up on test results, but this was not done in this instance.
