Summary

The facility failed to establish and maintain an effective infection prevention and control program, as evidenced by staff not adhering to Enhanced Barrier Precautions for a resident requiring such measures. Specifically, two staff members, identified as Unknown A and Unknown B, did not wear the appropriate personal protective equipment (PPE) while providing direct care to a resident with a tracheostomy who was on Enhanced Barrier Precautions. Video monitoring screenshots showed that one staff member did not wear a mask or gown, and the other did not wear a gown during high-contact care activities. The resident involved had significant medical conditions, including anoxic brain damage, respiratory failure, type 2 diabetes with hyperglycemia, Alzheimer's disease, chronic kidney disease stage 3, tracheostomy status, persistent vegetative state, unspecified dementia, and aphasia.
