The infection control logs did not reflect any COVID-19 cases during the relevant period, and staff reported not being in-serviced on COVID-19 protocols in the preceding 60 days. The facility's infection prevention and control policy required the identification and management of infections, implementation of isolation precautions, and staff education, but these procedures were not followed in this instance. The administrator and DON both acknowledged gaps in communication and documentation, including the failure to obtain and review hospital records in a timely manner. The lack of adherence to established infection control protocols and failure to isolate the COVID-positive resident or protect the roommate constituted a deficiency in the facility's infection prevention and control program.
