Summary

The facility failed to provide the required minimum of 80 square feet per resident in multiple occupancy rooms, as observed in rooms 109, 111, and 117. Each of these rooms contained three resident beds but measured only 231.6 to 236.4 square feet, which is less than the required space per resident. This deficiency was identified through a review of the facility's Request for Waiver Variation Letter and confirmed by direct measurement during multiple observations.
