Summary

Several staff members reported not receiving any training on the facility's infection control policies or procedures, and agency staff in particular noted a lack of orientation or instruction regarding these protocols. Additionally, the facility did not maintain a comprehensive process for tracking, trending, or analyzing infection data as required by its own policies. Review of the infection control documentation revealed no evidence of infection tracking or data analysis for the period reviewed. The DON and IP both confirmed the absence of a comprehensive infection surveillance and analysis process, despite expectations that such a system should be in place.
