The facility's policy requires that each resident receive care and services to maintain the highest practicable physical, mental, and psychosocial well-being, including assistance with activities of daily living such as grooming and personal hygiene. The resident, who has diagnoses including morbid obesity, urinary tract infection, hypokalemia, and hypothyroidism, reported not receiving scheduled showers for several weeks. During an interview, the resident expressed concern about greasy and knotted hair, which was also observed by surveyors. The DON confirmed that the resident did not receive baths or showers as scheduled and preferred during the review period.
