Summary

The facility failed to develop and implement care and services consistent with professional standards of practice to prevent the development of a pressure ulcer, resulting in actual harm to a resident who developed a Stage II pressure ulcer on the right heel. The resident, who had a history of dementia and cognitive deficit, was readmitted after a hip fracture and surgical repair, which led to a significant decline in mobility and increased dependence on staff for bed mobility and transfers. Despite these changes, the care plan was not updated to reflect the resident's increased risk for pressure ulcers, and individualized interventions were not implemented.
