The facility's failure to document and act upon these repeated concerns, as well as to implement safety restrictions or protections as outlined in their own policies, resulted in the resident being subjected to ongoing inappropriate contact and a lack of necessary services to ensure her safety. The deficiency was confirmed by the Nursing Home Administrator and Director of Nursing, who acknowledged that the facility did not provide the required services or protections for the resident with dementia.
