The facility failed to develop and implement an accurate and comprehensive care plan for a resident with multiple diagnoses, including schizophrenia, type two diabetes mellitus, and cerebral infarction. This omission was confirmed during an interview with the MDS RN, who acknowledged the lack of incontinence-related interventions in the resident's care plan. The facility's policy requires that care plans be comprehensive and person-centered, with measurable objectives and timetables to meet each resident's needs, but this was not followed in this case.
