Summary
The facility failed to ensure that a follow-up to a pharmacy recommendation was completed for Resident #51. The contracted pharmacist recommended a review of the resident's antipsychotic medication for appropriate diagnosis and necessity. In response, the resident's primary care physician requested a psychiatric evaluation to confirm the diagnosis and need for the antipsychotic medication. However, a review of both electronic and hard copy charts revealed no evidence that the psychiatric evaluation was ever completed. This was confirmed by the Director of Nursing during an interview, verifying that the evaluation was not performed as instructed.
