Failure to Maintain Safe Temperatures and Boiler Maintenance

Summary

The facility failed to maintain comfortable and safe temperature levels in residents' rooms and common areas, as required by State and Federal regulations. Six resident grievances were filed over several days regarding lack of heat, and observations confirmed that temperatures in all sampled rooms, corridors, and stairwells were below required ranges. The Administrator was not aware of the loss of heat in the building during this period. Additionally, there was no documented evidence that the boiler room equipment and Packaged Terminal Air Conditioner (PTAC) units were routinely maintained. Despite this, the Administrator denied receiving any such communications. These failures resulted in Immediate Jeopardy due to the likelihood of more than minimal harm to all residents.
