Facility staff failed to consistently document the Activities of Daily Living (ADL) status and care provided to a resident with severe cognitive impairment and multiple medical conditions, as required by professional standards. The resident, who had diagnoses including Alzheimer's disease, dementia, diverticulitis, and reduced mobility, was dependent on staff for assistance with eating and bed mobility. The care plan identified risks such as malnutrition and skin breakdown, and noted the resident was sometimes resistive to turning and positioning.
