Summary

The facility failed to ensure the accuracy of a Minimum Data Set (MDS) 3.0 assessment for one resident. The resident, who had a diagnosis of type one diabetes mellitus with diabetic chronic kidney disease, was prescribed and administered Eliquis, an anticoagulant, for deep vein thrombosis (DVT) prophylaxis. Review of the resident's Medication Administration Record (MAR) confirmed that the medication was given consistently throughout the month. However, the MDS assessment completed for the resident did not indicate that the resident was receiving an anticoagulant in Section N, which is designated for documenting high-risk medications.
