Summary

Nursing staff failed to perform appropriate hand hygiene and change gloves during the provision of care for a resident who was dependent on staff for toileting and personal hygiene, incontinent of bowel and bladder, and had open wounds. Staff provided incontinent care, handled soiled linens, and touched various surfaces without removing gloves or performing hand hygiene as required by facility policy. The Director of Nursing and Administrator both stated that staff were expected to follow hand hygiene and EBP policies, but observations and interviews demonstrated that these protocols were not consistently followed.
