Interviews with facility staff, including the Activity Director and Assistant Director of Nursing, revealed a lack of communication and follow-through regarding the residents' complaints. The Activity Director acknowledged the difficulty in planning activities for a diverse age group and confirmed that concerns discussed in resident council should be written up as grievances, but this was not consistently done. The Assistant Director of Nursing did not recall being informed about the noise complaints. All residents involved were cognitively intact and had voiced their concerns clearly, but the facility failed to document and address these grievances according to policy.
