Summary

The facility failed to ensure proper hand hygiene and use of personal protective equipment (PPE) for multiple residents under Enhanced Barrier Precautions (EBP). For one resident with an indwelling catheter and pressure ulcers, a nursing assistant donned gloves and a gown but continued to wear the same surgical mask throughout the day, contrary to posted EBP instructions.
