Summary

A resident with diagnoses including muscle weakness, pain, high blood pressure, and heart failure was dependent on staff for assistance with personal hygiene and grooming, as documented in the care plan and Minimum Data Set (MDS) assessment. The resident was observed on two consecutive days to have visible chin hairs up to 1/2 inch long and expressed dissatisfaction with the presence of chin hairs, stating a desire for staff assistance with shaving.
