During an interview, the Director of Social Services, who serves as the facility's Hospice Liaison, and the Administrator confirmed that the care plan did not identify the resident's specific end-of-life wishes or preferences. The surveyor pointed out these omissions, and both staff members acknowledged that the plan failed to include individualized information necessary for providing person-centered Hospice care.
