Summary
Facility staff failed to ensure that nurse staffing information was accurately posted and kept up to date on the staffing whiteboards for both nursing units. During multiple facility tours, surveyors observed that the whiteboards displayed outdated staffing information from previous days, lacked required staffing ratios, and in one instance, did not include the names of Geriatric Nursing Assistants on duty. Additionally, staffing information was not posted at the facility entrance as required.
