Summary
The facility failed to complete a Significant Change Minimum Data Set (MDS) Assessment within 14 days for two residents who transferred hospice services. One resident, admitted with multiple myeloma, transferred to a new hospice company on 03/18/2025, but no significant change MDS assessment was submitted at that time.
