Summary

The facility failed to develop a comprehensive, person-centered care plan for a resident with a Level II PASRR determination. Record review showed that the resident was admitted with diagnoses including Schizoaffective Disorder, Depressive Type, and had been approved for admission by the Level II Authority for a specified period. The resident's OBH-PASRR Evaluation Summary and Determination Notice indicated the presence of a serious mental illness and recommended nursing home admission.
