Summary

The facility failed to offer influenza vaccinations to four out of five residents reviewed for immunizations during the 2024-2025 influenza season, despite having a policy in place requiring annual influenza immunizations for residents. Clinical record reviews showed no documentation that these residents, who had various diagnoses including heart failure, bipolar disorder, diabetes, seizure disorder, dementia, and muscle weakness, were offered the influenza vaccine. The residents' cognitive statuses ranged from intact to severely impaired, as indicated by their BIMS scores.
