Summary

The facility failed to ensure that State Survey Results were readily available for residents and visitors to view on three out of five days during the survey period. Observations revealed that there was no posting near the front entrance indicating the location of the survey results, and on one occasion, a sign in the hallway by the therapy department directed individuals to a wall pocket where the results were supposed to be, but the documents were not present.
