Summary

The facility failed to ensure the accuracy of a resident's Minimum Data Set (MDS) assessment by incorrectly documenting the resident's PASRR (Preadmission Screening and Resident Review) status. The resident, who had multiple diagnoses including schizoaffective disorder-depressive type and generalized anxiety disorder, was admitted with documentation of both PASRR Level I and Level II screenings that identified major mental illness and indicated a need for specialized mental health services. Despite this, the Admission MDS Assessment recorded the response to section A1500, which asks if the resident is considered by the state Level II PASRR process to have a serious mental illness or related condition, as 'no.'
