Facility policy required clinicians to determine and document tobacco status, complete smoking assessments on admission and quarterly, and develop individualized care plans. However, there was no policy available regarding how to complete a smoking assessment, and staff interviews revealed inconsistent practices and lack of direct observation by licensed nurses. These actions and inactions resulted in the failure to provide adequate supervision and a safe environment for the resident during smoking activities.
