A resident with a history of weight fluctuations, advanced age, acute kidney failure, and morbid obesity was receiving Lasix for fluid retention. The last nutritional assessment for this resident was completed on November 27, 2024, and the next quarterly assessment was due before the end of February 2025. However, the RD did not assess the resident during this period, despite being present in the facility in early March. Interviews with facility staff, including the Dietary Services Supervisor and the Quality Assurance Nurse, confirmed that the facility's policy requires the RD to assess residents on admission, quarterly, annually, and as needed.
