Summary

The facility failed to protect two residents from physical abuse by another resident. In the first incident, one resident was observed by a CNA to strike another resident in the face and push her to the floor as they crossed paths in the hallway. The aggressor was described as having a history of anger outbursts and being aggressive toward both staff and other residents, with previous attempts to physically harm staff. The victim sustained redness to the left side of her face as a result of the assault. Documentation and witness interviews confirmed the unprovoked nature of the attack and the aggressor's inability to be easily redirected.
