Summary

The facility failed to ensure the accuracy of Minimum Data Set (MDS) assessments for three residents. For one resident, the MDS inaccurately documented that the resident had adequate vision without corrective lenses, despite both the resident's son and a registered nurse confirming the resident's vision was impaired and required corrective lenses.
