Summary

The facility failed to ensure that a resident who was unable to carry out activities of daily living received the necessary care and services to maintain good personal hygiene. Specifically, a resident who required extensive assistance with personal hygiene and was dependent on staff for showers and bathing was observed with long fingernails and a contracted left hand. The facility's policy required nail care as part of routine activities of daily living, and the resident's care plan included an intervention to trim nails weekly and as needed. However, observations revealed that the resident's fingernails were long, stained, and curling into their contracted left hand, indicating a lack of adherence to the care plan. Interviews with facility staff further highlighted the deficiency.
