Interviews with various staff members, including the Infection Preventionist, LVN A, and the NP, indicated a lack of clarity and adherence to protocols for post-fall assessments. The DON and ADM were also unable to provide documentation of the required assessments, and there was no evidence that the facility staff monitored the resident for changes in condition as expected. This failure to follow established protocols and document care placed the resident at risk of not receiving necessary medical care and potentially exacerbated her injuries.
