CNAs and nurses did not consistently report or document skin changes, and the resident missed a scheduled wound care appointment due to being out of the facility for another medical appointment. The facility's Director of Nursing and Administrator acknowledged the expectation for regular skin assessments and timely interventions, but these were not effectively implemented, leading to the resident's deterioration and subsequent hospitalization.
