The facility failed to ensure that a call light was within reach for Resident #52, who had moderate cognitive impairment and required assistance with activities of daily living (ADLs). During an observation, the call light activation button was found on the floor and behind the nightstand, making it inaccessible to the resident. This oversight was verified by a Corporate Registered Nurse. Resident #52 had significant diagnoses including unspecified head injury, dementia, and cognitive communication deficit, and was on hospice care with scheduled toileting to promote continence.
