The facility failed to maintain respiratory care equipment and provide respiratory care per physician orders for three residents. Resident 15's oxygen concentrator was observed with a filter covered in dust, indicating it had not been cleaned as required by the facility's policy. Observations showed the oxygen concentrator's flow rate was set at 2.5 liters on multiple occasions. An LPN and the Regional Nurse Consultant (RNC) confirmed the incorrect setting. The RNC verified the physician's order and acknowledged the discrepancy. These failures in maintaining proper oxygen flow rates and equipment cleanliness placed residents at risk for respiratory complications.
