The Director of Nursing acknowledged potential issues with communication due to nurse managers' availability, and the medical doctor confirmed a lack of updates regarding the hospice patient's condition. The facility's failure to adhere to its policies and ensure timely communication and documentation contributed to the deficiencies identified in the report.
