Summary

The facility failed to provide comprehensive behavioral care management for a resident with severe cognitive impairment and a history of aggressive and wandering behaviors. The resident, diagnosed with Alzheimer's disease, dementia, and adjustment disorder, exhibited aggressive behaviors towards other residents and staff, resulting in two resident-to-resident incidents. Despite these behaviors, there was a lack of documented social work visits, supportive interventions, or follow-up actions to address the resident's needs, including communication barriers and grief over the recent death of their son.
