However, a review of Resident #5's electronic medical record revealed 133 days of missing 2-hour charting and several entries recorded at the exact same time, indicating that staff did not properly document whether the checks were completed. Resident #6, admitted with diagnoses such as Parkinsonism and diabetic neuropathy, also required 15-minute visual safety checks following a fall.
