Summary

The facility failed to ensure the completion and accuracy of Section C (Cognitive Patterns) of the Minimum Data Set (MDS) assessments for seven current residents and one discharged resident. These residents, who had various diagnoses including chronic respiratory failure, diabetes mellitus, dementia, quadriplegia, and spinal stenosis, were not properly assessed for their cognitive status.
