The facility failed to provide meals according to the established menu and did not respect residents' dietary choices during a recertification/complaint survey. The Certified Dietary Manager (CDM) explained that residents typically receive the Regular meal unless they request an alternative. However, during the lunch service, the District Manager (DM) altered a meal ticket to substitute a hamburger for the Dijon Pork Loin, which was the Regular meal option. This substitution occurred because the facility ran out of pork loin due to over-portioning by a dietary staff member. As a result, 10 residents who had chosen the pork loin did not receive it and were instead given the alternative meal without being informed or given a choice.
