Interviews with facility staff revealed that the MDS coordinator, with assistance from nursing staff, was responsible for completing the baseline care plans. However, the plans were incomplete and lacked personalization to the residents' specific needs, leading to deficiencies in care planning. The social services director and regional social services director confirmed that the areas they completed were included in the comprehensive care plan, but the baseline care plans remained insufficient in addressing the residents' immediate needs upon admission.
