The Treatment Nurse (TN) and Infection Prevention Nurse (IPN) confirmed the absence of documentation clearing the resident from contact isolation, emphasizing the need for proper signage to prevent infection spread. The Director of Nursing (DON) also stated that residents on transmission-based precautions should be reassessed and cohorted properly to ensure the safety of other residents. For Resident 10, who was on EBP due to a gastrostomy tube and risk for multidrug-resistant organism infection, a CNA (CNA 4) was observed changing the resident's linen while only wearing gloves, without the required gown. The CNA acknowledged the oversight and the importance of wearing the full PPE during high-contact activities. The Infection Prevention Nurse (IPN) and the Director of Nursing (DON) reiterated the necessity of wearing the required PPE to prevent the spread of infection, as outlined in the facility's policy.
