The facility failed to provide necessary behavioral health care and services to a resident, who was admitted with multiple diagnoses including epilepsy, chronic kidney disease, chronic congestive heart failure, and type II diabetes mellitus. The resident's cognitive skills were moderately impaired, requiring maximum assistance for activities of daily living. Despite having a care plan in place to address potential verbal aggression related to ineffective coping skills, the facility did not document behavioral monitoring or assessments for the resident's ineffective coping skills. This lack of adherence to the policy contributed to the deficiency in providing necessary behavioral health care and services to the resident.
