Summary

The facility staff failed to ensure that a Pre-admission Screening and Resident Review (PASARR) was completed prior to admission or shortly thereafter for four residents. Resident #49 was admitted with diagnoses including major depressive disorder with psychotic symptoms, anxiety disorder, and PTSD. Despite being on psychotropic medications, no PASARR was completed prior to admission, and the subsequent attempt by a newly hired social worker was incomplete and contained errors. The facility lacked a social worker for several months, and the new social worker did not meet the required qualifications.
