The dietary manager, who had recently assumed the role, and the dietician, who visited the facility twice weekly, did not monitor meal tray preparation, contributing to the oversight. Specific residents were affected by these deficiencies. Resident #86, with severe cognitive impairment and on a pureed diet, did not receive the appropriate vegetables as per the menu. Similarly, Resident #25, also with severe cognitive impairment and on a pureed diet, did not receive pureed green beans or a replacement.
