Summary

The facility failed to provide sufficient nursing staff to meet the needs of its residents, as evidenced by the inadequate incontinence care and personal hygiene maintenance for two residents. Additionally, the facility failed to ensure adequate supervision and care on the memory care unit, which was usually staffed with only one CNA. This staffing level was insufficient to meet the residents' needs for meals, incontinence care, showers, safety monitoring, and activities. Observations showed a resident in the memory care unit yelling for help and shuffling out of the bathroom with pants down, while the sole CNA was occupied with other residents. Interviews with staff confirmed that one CNA was not enough to provide the necessary care and supervision for the memory care unit residents.
