Facility Fails to Ensure Accessible Environment for Residents

Fall River HealthcareFall River, Massachusetts Survey Completed on 02-05-2025

Summary

The facility failed to accommodate the needs of residents on one of its units by not ensuring that the physical environment was accessible. Specifically, the doors to the River 1 Unit were consistently closed, making it difficult for residents in wheelchairs to open them independently. Multiple observations were made of residents struggling to open these doors, with some resorting to kicking them open or relying on other residents or staff for assistance. The residents expressed frustration and concern during a Resident Group meeting, noting that the doors were always closed without a clear reason, and they feared the doors might close on them if they did not move quickly enough. Additionally, the facility did not maintain the handicapped switches for the doors leading to the smoking area in good repair. The surveyor observed that the handicapped buttons were not functioning, and the outside button was loose and tilting. Residents reported that the buttons had been broken for months, requiring them to rely on others to hold the door open. The Maintenance Director acknowledged that the outdoor button had been non-functional for over a year, and the issue was only addressed after the surveyor's inquiry. Interviews with staff, including the Director of Nurses and Corporate Nurse, revealed a lack of awareness regarding the reasons for keeping the Unit 1 doors closed and the non-functioning handicapped buttons. There was no infection control or safety rationale provided for the closed doors, and no facility policy was in place to address the accommodation of residents' needs. The deficiency highlights the facility's failure to ensure an accessible environment for residents with mobility issues, impacting their independence and quality of life.

F0558 F558: Reasonably accommodate the needs and preferences of each resident.
Short Summary

Call lights were not kept within reach for multiple residents who needed staff assistance. Call Light Not Kept Within Reach
D
F0558 F558: Reasonably accommodate the needs and preferences of each resident.
Short Summary

A resident with severe cognitive impairment, wheelchair use, lower-extremity impairment, and dependence for most ADLs had her call light repeatedly observed on the floor out of reach while lying in bed. Her care plan directed staff to keep the call light within reach and encourage use of the bell for assistance, and multiple staff members stated call lights should always be within reach and that staff were responsible for placement. The resident's family member said she could use the call light, but survey observations showed it was not accessible during several checks.

Closet Access Blocked by Bed Placement
D
F0558 F558: Reasonably accommodate the needs and preferences of each resident.
Short Summary

A resident with bilateral knee replacements and difficulty walking could not independently access the closet in the room because the resident’s wheelchair would not fit between two beds placed footboard-to-footboard. The resident said staff had been told multiple times, but the room layout was unchanged; staff gave conflicting accounts of the resident’s mobility, and the Administrator and Maintenance Director confirmed the wheelchair could not fit between the beds.

Call Light Not Within Reach for Resident at Risk for Falls and Seizures
D
F0558 F558: Reasonably accommodate the needs and preferences of each resident.
Short Summary

A resident with seizures, aphasia, dementia, and a recent fall with injury did not have his call light within reach while in bed. The care plan directed staff to keep the call light in reach, but surveyors observed it wrapped around a wall-mounted switch box above the head of the bed and out of reach. The resident and an NA confirmed it could not be reached, and the record showed a prior fall after the resident tried to transfer without using the call light.
